Parent Consent form for Weekly-Update videos

The cast of You're a Good Man, Charlie Brown have asked if they could make their weekly update videos public so that family, friends, and others may view them. 

If you agree to make your child’s video available to the global community, for safety reasons, only your child’s first name will be published. 

If you agree to allow your child’s work to be made public in the manner described above, please complete the form below, sign it,  and send it back to me as soon as possible.
